The team includes organizations such as Zorgbalie, Vrij CLB, KDV Speelboom, Huis van het Kind Aalst, Wiegwijs, Kind en Gezin, Team START (early diagnostics), 1G1P RTJ De Tafels, ELP, Envia Dender vzw, MensenvoorMensen, Home-Start, and Advies & Coaching ADS.
The collaboration has three main objectives: seeking fast and accessible help within basic care, involving organizations that can play a role in the request for help, and offering additional care if needed.
Conditions for participation include the family's consent for case discussion, the referrer remaining the point of contact for the family and involved partners, and active feedback to the family and the functional team.
Professionals can submit their cases during online consultation moments on Fridays from 9 to 10 AM. This requires filling out the registration and consent form together with the family.
