If you prefer to fill out the test on paper or need assistance, you are welcome by appointment at the De Schakel community center (Oudstrijdersplein 1, 2547 Lint) on Monday, November 9th from 9:30 AM to 12:30 PM, and on Tuesday, November 10th from 2:00 PM to 4:30 PM.
If the test indicates a score of 12 or more, it is recommended to schedule an appointment with your GP for a targeted check-up via a simple blood test.
